General/Base Information:

Birth Name: (The name they were legally given at birth by their parents)
Name Origin: (Why was the name chosen? Where did it come from? Does it have a country of origin? What does it mean? Or was it made up out of the blue and means absolutely nothing? Repeat for name changes, nicknames, and alias’ if they are applicable)
Current Name: (Have they always kept the same name? Did their name change? If so, what did it change to? Nicknames don’t count as a name change.)
Name Change Reasons: (Why did they change their actual, legal name?(if applicable))
Nickname: (An alternate name that they are sometimes, or prefer to be called. This can be a shortened version of their already existing name(“Sam” instead of “Samantha”) or a different name entirely unrelated to theirs, but that fits them or they are commonly called)
Alias: (NOT to be mistaken as a “Nickname”. A false name given to protect ones identity, such as when a person gives a fake name when arrested, or a detective working a case gives a fake name and pretends to be someone else when trying to find out information)

Birth Date: (What day, month, and year was your character born?)
True Age: (How old is your character?)
Appeared Age: (Does your character look their age? Do they look older than they are? Younger than they are? How much older/younger do they look? What makes them look older or younger than they are?)

Desired Gender: (Is your character content with their gender? Would they rather be the opposite gender? Is it just an idle thought, or have they actually tried to dress like or become the opposite gender?)

Species: (Species is typically the largest umbrella category that will determine your character's Race. For example, Human is a species, while African, Caucasian, Indian, et cetera would be Race. Elf would be a seperate race from human, while there might be different races of elf, such as forest elf or field elf. Wolf might be the main species while Grey Wolf, Red Wolf, Artic Wolf, et cetera might be the Race/Sub-species/breed)
Race: (What race is your character? Note: “White”, “Black”, “Tan” are NOT Races, they’re “skin colors”. Norwegian, Swedish, German, Finnish, Polish, French, Irish, Scottish, Swiss, et cetera would be an example of "White"; races. African, African-American, Egyptian, et cetera may be examples of “Black” races. “Tan” races could be a long list of things – Mexican, Pacific Islander, Arabic, Indian, Vietnamese, Native American, et cetera may be “Tan”(depending, some are lighter or darker in every race).
Sub-Race: (This might be a secondary or later-developed race such as a Werewolf or Vampire, while Race would be their race of origin or location such as African, Chinese, Caucasion, et cetera. Likewise, if you have a shapeshifting creature with multiple forms, the main race might be something such as a dragon, while the "sub-race" or secondary race/species might be human, if they take on a human form)

Nationality: (What country is your character legally a citizen of?)

Skin Tone: (This can range anywhere from “Albino”/really pale/pasty all the way to skin that legitimately is “black”, or somewhere in-between like ashen(kind-of-grey tinted skin, like when deprived of too much sun), sunkissed(very barely tanned), to mocha, to semi-dark. Whatever suits your fancy.)

Natural Hair Color: (Pretty self-explanatory. If going for realism, people are born with any shades or variations of black, sometimes gray, brown, blond, red/orange/ginger, and platinum/white. If you don’t care about realism…well then, go wild.)

If you don’t care about realism, then just go wild!(just please, for the love of whatever diety you worship, steer clear of the rainbow eyes. Not nearly as attractive as you might think. Blek. Muscle Mass: (Is your character athletic and muscular? Or are they lazy and flabby? Keep in mind, a character who loafs on the couch eating Bon-Bons and potato chips all day, every day isn’t going to be a skinny, muscular super-model and athlete, they have to actually get up, eat healthy, and work out!)

Skills and Talents:

Note: Not all "talents" or "skills" have to be something like art, music, or sports. Here's an extensive list of possible talents, and it's not limited to ONLY these things either: www.michaelhartzell.com/Blog/b…
Natural Talents: (“Talents” doesn’t mean a character automatically knows how to do something. A “Talented” artist doesn’t just pick up a pencil at the age of two and draw Leonardo Da Vinci’s works perfectly in a few days. Talent just means it’s easier for them to learn over time without as much guidance. Talents can also be undiscovered by the character, but still exist.)
Learned Skills: (Can overlap with Talents, but doesn’t have to. Any skill that a character has worked at trying to be good at, and knows how to do.)
Hobbies: (Things that the character may enjoy doing on the side, but doesn’t necessarily always focus on it, or plans on making it into a career or taking it seriously. Basically, casual skills, like doodling for fun, or sewing just because it relieves stress)
Forever Failed Skills: (Anything the character isn’t good at, or can’t do, no matter how hard they try. Everyone has something they can't do.)

Checks and Balances:

Greatest Strengths: (Your character’s BEST, BEST traits. Traits that are probably obvious to everyone else. It can be anything; skills, appearances, intelligence, reputation, health, whatever! Just try not to go overboard, try to be reasonable and realize when enough good traits is enough. Your character has awesome traits, it doesn’t mean they’re “Perfect”)
Greatest Weaknesses: (The downfalls of your character, those things that no one wants to have, but that doesn’t mean they don’t(Anyone who tries to take this part out or say their character has no flaws or weaknesses is going to get slapped in the face with a chainsaw). Some traits may be easily visible to others, some are more hidden away, but they must be significant. It can be anything; skills, appearances, intelligence, reputation, fears, health, whatever! Try and balance it out about-evenly with good traits.)

Personality Development:

Personality: (How does your character behave, think, and feel? What are their positive personality traits? What are their negative ones? How do they react to others? To being alone? Do they act differently in public than in private? Do they act differently on the surface than how they feel inside? Are they honest? Or liars? Why do they have the personality and beliefs that they do?)

Fears: (What are they afraid of? Are these fears minor, or serious/phobic? How do they respond to these fears? For example: an arachniphobe. Do they get a little edgy, and go get a shoe to squish the spider with? Or do they scream, hyperventilate, and demand someone else squish it for them because they’re too scared to get close to it? Are they SO fearful that they take extreme measures to prevent experiencing these fears? Are they on medication for some of their phobias? Are they seeking special treatment to get over their phobias? Have they had a huge impact on how they live their lives, or how they function in the real world(Such as a person with Agraphobia, the irrational and extreme fear of people, may stay home and not leave most of the time to interact with other people or work a job/school)?)

Sexual Orientation: (Heterosexual/straight? Gay? Lesbian? Bi? Pansexual? Asexual/Abstinent? Etc)
Relationship Interests: (What is your character looking for in a relationship? Committed, loving, life-long relationship with a loyal, nice guy? A risky bad-boy who’s into all kinds of “interesting” things, and living on the wild-side, but maybe not incredibly loyal? Uncommitted, “open” relationships? One-night stands?)
Type Turn-On’s: (What types of people, physical or personality-wise, is your character attracted to or prefers? Why?)
Type Turn-Off’s: (What types of people, physical or personality-wise, is your character really NOT attracted to(on an exclusively romantic level)? Why?)

Social Standing: (What type of person is your character, socially? Are they that really popular person surrounded by tons of friends and partiers? Are they semi-liked and have a good number of friends, but maybe not SUPER-popular? Are they very exclusive, and don’t have many, but very good friends? Maybe they’re a loner, and don’t like others?
Friend Interests: (What type of people does your character like to hang around? Outdoorsy, sports-type people? Cheerleaders? Book-worms? Scene/Emo/Goth type of kids? Gangsters? Nobody? Etc. Why?)

Do they simply claim their religion to be “one with the crowd”? Are they simply following a religion because it’s their parent’s religion, or societally expected, but they don’t want to be a part of it?

Absolute Loves: (No, I don’t mean people. I mean their absolute, favorite things that they love to do excessively, be around, think about, and learn about. Such as someone might be obsessively into horses, or perhaps into fixing up cars and tinkering with technology.)

Dislikes: (What are the things that annoy or anger them, but not as much as their Absolute Hates? What do they try to avoid, or show disgust towards? Why do they hate these things?)
Hated Foods: (What sort of foods can they not stand? That disgust them or that they just don’t particularly like the taste of?)
Disliked Media: (Do they hate television? Music? Video games? Do they not like books, or tabletop board games, or other real-life activities? What, out of those media(types/genres of shows, movies, music, video games or consoles, etc.) do they dislike? Why?)

Are both their parents alive? Does your character like or dislike them? (Try not to put “Unknown” for either one if it can be avoided(especially/at least with major characters, though minor/background/"Only-seen-once" characters without major roles you could probably get away with excluding it), even if deceased or not around, or not talked about, as it usually only shows laziness in character development. Edit, 3/22/14: Added a new section called "Voice Quality" between Clothing Type and Alliances, which includes Accent, Pitch, and Inflection; suggested by elsie on CS

Edit, 11/18/13:
No you don't HAVE to download to use, it's just for those that feel like being nice and paying me to use it :D

Also! Springhole.net has a new guide that might be able to help you with character creation, here: Character Creation & Development Theory
(Or, How To Make Characters 101!)

I highly recommend it!

==================================

Edit, 10/9/13: Decided to make it downloadable For Points. The download includes the html brackets so you don't have to manually go through and add in bold and underline code yourself. I guess you don't really have to download for to get any of the content, more just for those that want to be generous and give some points for when they decide to use the form I created ;P

Trying to save up for a Premium Membership :dummy: It's only about a dollar per download
====================================
Edit, 10/1/13: Added a few questions and a category.
:pointr: Added a new section, Addictions, between Medical Record and Education categories.
:pointr: Added a new question, Hygeine, to the Physical Appearance category.
:pointr: Added a new question, Virginity?:( :lmao: ), to the Romantic Relationships category.

Totally just learned how to do larger font size/headings. OTL

Had this up on CS and Noveljoy and figured it was time to put it up on DA for anyone who might be interested in it. I know I have a few watchers that are either already full-realized writers, or wannabe writers. Whichever side of the fence you're on, or maybe somewhere inbetween, I figured this would be a help for creating well-rounded characters for anyone that wants to create characters, whether they be for stories, roleplaying, or even just drawn characters only for yourselves.

Spent literally days trying to think up every aspect possible on character creation for creating, revamping, or simply adding more detail to characters when I first made this, and (though I haven't applied them to the form yet) have thought up more character-building aspects since then to add on. I tried to include a lot of things that most people DON'T consider when creating their characters, but it's not required to use every section of the form.

It's free to use for anyone, you don't need to ask to use it, and I'm open to suggestions for additions to further improve the form if anyone has any ideas of things to add. Mainly this was made with humans in mind, but feel free to modify for other species or animals as you see fit, or add your own categories as needed.

Rules of Use

:bulletred:You don't need to ask to use it. You may use it for anything you wish, with exception to making a profit off of the blank form/guide itself through points or cash. If you re-distribute the blank form and share it with others, it must be for free.

:bulletred: You don't need to pay points to use it. That option is, well, optional! You can simply copy and paste for free. If you download for points, the benefit is you won't have to add coding tags for formatting yourself manually.

:bulletred: You may re-distribute this form to other sites, but it would be nice if you could give credit to the original posted here.

:bulletred: You may edit this form to fit your needs as you see fit(i.e. add new sections, or take sections away, or edit them to work for creating an animal instead of a human, etc.)

:bulletred:You don't have to link me anything when you use it. I probably won't read most of them anyway, unless you want a critique/help on your character build(see rule below this). Nothing personal.

:bulletred:Feel free to ask me to critique your characters if/when you fill out the form, or help on improving your characters. I can't guarantee I'll have the time, but you can ask. NOTE: DO NOT ASK ME TO CRITIQUE YOUR CHARACTER OR GIVE YOU ADVICE IF YOU DON'T WANT ME TO TELL YOU WHAT'S "WRONG" WITH YOUR CHARACTER AND FLAME ME. I'm not going to sit and say "Wow, this character is crap. What were you thinking?", but I might tell you "This power you gave your character is ridiculous and doesn't make sense", or "Your character has too many strengths and not enough weaknesses, it's overpowered" or something, and suggest how you can change it to make it better(doesn't mean you HAVE to change it, but that doesn't mean I won't tell you what's wrong with it either). If you can't handle an honest critique and are only looking for an ass-kissing about how awesome your character is, no matter how bad or ridiculous, DON'T ASK FOR A CRITIQUE. If you don't like what I have to say, maturely tell me that you don't want to have me critique your character anymore, or retain your silence, and go find someone else to tell you what you want to hear.

Overall I love this template, its just that 'Hermaphrodite' is an outdated and, to some, an offensive term. Also, regarding gender, you should use words like 'Sex' and 'Gender Identity', as sex and gender are different. I just wanted to point this out so that people don't get hurt.

I do really like how this template is organized and how easy to use it is! I also like how you elaborated on the different sections and gave examples, that helps a lot! I really like how you gave a link to the BMI calculator for people to use too!
